There can be 2 meanings to your question.
The answer to the second question, I am sure you are aware too, is that, in abroad, they do not recognise medical qualification from India. So you would have to apply for an exam and get yourself licenced.
As far as after 12th is concerned, you would have to apply for MCAT exam and then appear for the interview and once you clear, make the necessary arrangements. As far as the google search reveals, most of the medical schools in Canada prefer in-house students and very few give chances to students away from their provinces. So before applying, you would have to check the institutes website and then only apply.
Best would be to consult an education abroad consultant, who would be at a better position to guide you through the different institutes, procedures to apply etc.
