Sorry to say but you have compared two different field which have no similiarity and are pursued at different times.
MBBS is an undergraduate study and on completion you deal with patients health issues being a doctor. Whereas IAS is a governnent service dealing with the nations administrative and economy and other important national issue, pursued after one is a Graduate any field.
Please note you can pursue both the courses but would not do justice to either of the courses.
At the sametime as mentioned by the counselor above, you would not be able to dedicate enough time towards civil service exam preparations as it requures long hours of dedicated reading time and study time just like medical.
Thus I would suggest self introspect as to where your potentials and interest lies. Though an aptitude test would help you realis
