Good to see that you have understood and introspect yourself very well at such a young age.
Please note, this introspection itself will lead to a change in you.
At the same time, you are a class 9th student, the most crucial year of your academics. At this time, you are stressed and are filled with anxiety which leads to nervousness and hence forgetting.
So here I would suggest, get rid of your anxiety, as always remember what you study only will bring you results.
Every student needs to prepare themselves with notes. Please note the syllabus is too much for a student to focus on all chapters and every inch of the book. So whatever you decide to study see that you do not skip those study materials and give it your best shot.
eg. Say you have planned to study only 6 chapters of a subject out of 8. So make sure that from the 6 chapters you do not skip any part or inch.
Also when you study make a study chart. Make notes and easy markings that will help you recall what you have studied. Also note that every word is not important. SO highlight few words or at the side make a note of few words which are important to the answer.
Make a chart of the formulas and revise every time. Practice sums everyday for an hour, but based on formulas to see whether you understand the sum.
At the same time, get all your jobs done before you sit to study. Whether it is television or calling a friends or messaging. Complete it before you sit to study, and jeep away from distractions. Stress on healthy eating and give sufficient time for sleep.
Also avoid last moment discussions with friends and especially of the chapters you have not studied. Avoid group studies as much as possible. Make it a point to revise everyday.

It’s good that you have shared this problem here and you wish to improve in studies. First of all check your study methods. How do you study?
Do you read loudly or in your mind?
Do you try to learn a concept or mug it up?
Do you keep your mobile away while studying?
These are the few factors that matter a lot for concentration and learning. One should read in mind, make notes and revise in order to get best results.
Also meditation will help you a lot in increasing the concentration level. Start thinking positively about studying and own self. It will definitely act as a booster.

Forgetting in exam can be due to two reasons: Weak memorising power or lack of interest in the subject. If you are liking the subjects, then you should take efforts for increasing your memory power a little. Trying the techniques suggested by Ms. Deepti are very essential to be done immediately.
Do not lose heart and such situation may come in studying years, but they have to be overcomed.
Do not have any distractions like cellphone, computer, tv, friends, etc around you while studying.
