Dear Sabrina ,Its very unfortunate that your career and interests are not at all aligned. However, with your realization, you still have a chance to rectify. Though you have put in 1+yrs., I feel , 1 or 2yrs. here-an-there will not make much of a difference when we try to lay the foundation stone of our career. So, not all is lost.

Good that you have realised it pretty soon, you still have time to think over your choice.
But before opting out, I would like you to re-think. Since you have cleared the MBBS entrance exam which in itself is an achievement.Is it because you failed in the 1st year, you are getting least interested to pursue it further.
If it is the above scenario, I would request you to go meet a personal counselor and take a session. Probably you are unable to deal with failure.
If not for the above, then yes, indeed, you would have to undergo an aptitude tests and based on the results make a confirmed choice.
